At Middlesex Healthcare Services, our Skilled Nursing program delivers advanced medical support in the comfort of home for individuals managing acute and chronic conditions. Licensed nurses provide care aligned with physician orders, focusing on accuracy, observation, and timely response to changes in condition.
Skilled Nursing provides licensed medical care for individuals who require ongoing clinical support at home. Services are delivered by Registered and Licensed Nurses and are provided under physician direction to address medical conditions that require professional oversight.
Skilled nursing may include assessment, monitoring, treatment, education, and coordination of care for individuals recovering from illness or surgery, managing chronic conditions, or requiring specialized medical interventions. The goal is to support recovery, promote stability, and help individuals remain safely in their home environment.
We provide the following skilled nursing services:
- Assessments, interventions, and teaching for acute and chronic diagnoses
- Surgical aftercare
- Medication instruction and management
- Wound and ostomy care
- Anticoagulation therapy
- Pain and symptom management
- Psychiatric and substance abuse nursing
Through collaboration with our interdisciplinary home care team, we are also able to provide:
- Physical Therapy (PT)
- Occupational Therapy (OT)
- Speech Therapy (SLP)
- Medical Social Workers (MSW)
- Home Health Aides (HHA)
A patient may be referred to Middlesex Healthcare Services by:
