Acute and chronic diagnosis management, surgical aftercare, and wound and ostomy care provided under physician direction.
Ongoing assessment and monitoring of medical conditions, coordination with physicians, and timely interventions to support recovery and stability.
Medication instruction and management, pain and symptom control, and psychiatric or substance-abuse nursing support as clinically indicated.
Support with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, mobility assistance, and basic health monitoring.

Adult Foster Care (AFC) is a program that supports adults who cannot safely live alone by pairing them with trained caregivers. The participant may live in the caregiver’s home or vice versa. Caregivers must not be a spouse or legal guardian.
Caregivers must be at least 18 years old and demonstrate responsibility and decision-making ability. They cannot be a spouse or legal guardian.
Participant:Individuals must be 16 years or older, have a medical or mental health diagnosis requiring daily supervision, and be eligible for MassHealth.
A designated nurse is available to provide guidance, answer questions, and support caregivers throughout the care process.
Eligible caregivers receive payment through our Caregiver Compensation Program.
