Home-based daily assistance supported through a state-funded program

Adult Foster Care is a MassHealth-funded program that provides structured, in-home support for individuals who cannot safely live alone. At Middlesex Healthcare Services, our Adult Foster Care program helps eligible individuals receive daily assistance while remaining in a familiar home environment.

Through coordinated care plans, caregiver guidance, and ongoing oversight, AFC supports both the individual receiving care and the caregiver providing assistance. The program is designed to promote safety, consistency, and independence while maintaining compliance with MassHealth guidelines.

Adult Foster Care pairs an eligible individual with a trained caregiver who provides daily supervision and assistance in a residential setting. The individual may live in the caregiver’s home, or the caregiver may live in the individual’s home. Caregivers must not be a spouse or legal guardian.

Participants in the AFC program may be as young as 16 years of age. While the program is often associated with seniors, it also supports younger individuals who require daily supervision due to medical or cognitive conditions.

Adult Foster Care services are guided by structured care plans and regular oversight. These plans outline daily routines, responsibilities, and the level of assistance required. Middlesex Healthcare Services works closely with caregivers to ensure program standards are followed and individual needs are addressed.

Regular communication and monitoring help maintain consistency in care while supporting the well-being of both the AFC member and the caregiver.

AFC services provide organized support through established program guidelines and clinical oversight. Caregivers receive direction and coaching to help ensure dependable assistance within the home.

Support may include:

  • Active supervision to ensure health and safety
  • Assistance with Activities of Daily Living (ADLs)
  • Medication reminders and routine support
  • Coordination related to daily schedules and appointments

Caregivers in the Adult Foster Care program are thoroughly screened and qualified as either a family member or a non-related individual. Caregivers must be at least 18 years of age and demonstrate the ability to make responsible decisions.

Caregivers are responsible for providing daily supervision, monitoring changes in health or behavior, and communicating with the Middlesex Healthcare Services AFC care team. Required documentation must be submitted in accordance with program standards.

Eligible caregivers receive a MassHealth tax-free stipend for the care they provide.

The AFC team at Middlesex Healthcare Services provides ongoing support, training, and coaching to both AFC members and caregivers, including:

  • Development of individualized care plans with clear goals and objectives
  • Monthly in-home visits by Registered Nurses and Care Managers
  • A 24-hour on-call support system
  • Services delivered by a CARF-accredited organization

Important Note: Adult Foster Care is a MassHealth-covered program and must be prescribed o authorized by the AFC member’s Primary Care Provider (PCP).

Eligibility for the Adult Foster Care program is determined separately for AFC members and caregivers.

AFC Member Eligibility Caregiver Eligibility

An individual may qualify for Adult Foster Care if they meet the following criteria:

  • Has MassHealth medical insurance
  • Requires assistance with Activities of Daily Living (ADLs)
  • Requires supervision for daily routines (at least 21 hours per day)
  • Has a medical condition or cognitive impairment that requires hands-on assistance or active supervision and coaching to complete activities such as bathing, dressing, toileting, transferring, and eating
Important Note: Adult Foster Care is a MassHealth-covered program and must be prescribed or authorized by the AFC member’s Primary Care Provider (PCP).
  • Must be at least 18 years of age
  • May be a family member, relative, or friend (not a spouse or legal guardian)
  • Must live in the same home as the AFC member and provide care there
